Alendronate (or alendronic acid) is a medication used to prevent and treat osteoporosis and other diseases of the bone.
It belongs to a class of drugs called bisphosphonates, which help increase bone density and reduce the risk of fractures. It's often prescribed to postmenopausal women and men with osteoporosis or those at risk of developing the condition.
Alendronate works by slowing down the breakdown of bones in the body. It helps to increase bone density and reduce the risk of fractures in people with conditions like osteoporosis. The drug binds to bone tissues, especially where bones are actively being broken down, called resorption sites.
By doing this, alendronate helps to maintain and strengthen bone structure. It's typically taken orally and needs to be taken on an empty stomach to ensure proper absorption.

For best results, it’s recommended that you take alendronate at the same time every day, on an empty stomach, with a full glass of water. Alendronate should not be taken in combination with other medications, vitamins, or supplements. To avoid risking unwanted side effects, be sure not to lie down for half an hour after taking alendronate.
Alendronate is often prescribed to be taken once a week because of its long-lasting effect on bone turnover. The medication works by slowing down the breakdown of bone (resorption) and increasing bone density over time.
The once-weekly dosing schedule helps maintain a consistent level of the drug in the body, providing continuous protection against bone loss and reducing the risk of fractures.
This less frequent dosing regimen can also improve adherence to the treatment plan, as it may be more convenient for patients compared to daily dosing.
The effects of alendronate start to happen after about one month of taking the medication. But, it may take up to 12 months for the medication to take full effect in bone protection.
Alendronate may remain in your body for many years after stopping the medication.
These drugs are not broken down in the body but are either removed through the kidneys or stored in the bones. The drug amount in the bones increases with use. Once in the bones, there's no known way to remove the medication.
How long the drug's effects last is still unclear. After taking alendronate for five years, markers showing bone activity stay low for at least another five years after stopping the medication.
It's essential to consult with a healthcare provider or pharmacist before taking any other medications while on alendronate. Some medications may interact with alendronate, affecting its effectiveness or increasing the risk of side effects.
You should never take an alendronate pill at the same time as other medications, vitamins, or even food.
Certain medications, such as calcium supplements, antacids containing aluminum or magnesium, and some oral medications, can interfere with the absorption of alendronate.
It's generally recommended to wait at least 30 minutes to an hour after taking alendronate before consuming other medications or supplements to ensure proper absorption.
Additionally, some medications may increase the risk of gastrointestinal side effects, such as stomach upset or irritation, when taken with alendronate.
The cost of alendronate can vary depending on several factors, including the dosage, brand, and pharmacy you visit.
The price for a generic version of alendronate typically ranges from $10 to $50 for a monthly supply without insurance. Brand-name versions like Fosamax or different formulations may be more expensive, sometimes costing over $100 for a monthly supply.
The most common side effects of alendronate include:
Less commonly, alendronate can cause more severe side effects, such as:
If you experience any of these severe side effects or have persistent or bothersome symptoms while taking alendronate, it's essential to contact your healthcare provider immediately. They can evaluate your symptoms and determine if any changes to your treatment plan are necessary.
